Pancreatic Tumor Surgery

With the direct care of the leaders of Zhongshan Hospital and the cooperation of relevant departments, we established the Pancreatic Tumor Center of Zhongshan Hospital affiliated to Fudan University with the pancreatic tumor specialists from the Department of General Surgery as the core members, integrating the services of Department of Radiology, Department of Radiotherapy, Department of Gastroenterology, Department of Endocrinology, Department of Chemotherapy, Department of Intervention Therapy and Department of Pathology. With the multi-department platform, we combine medical practice, teaching and research, aiming to integrate the advantages of these disciplines and further improve the diagnosis, treatment and research of pancreatic cancer. Currently, the key tasks of clinical practice are to: optimize the diagnostic and treatment procedures of pancreatic tumors through the coordinated operation of the platform, establish and adhere to the diagnostic and treatment standards for pancreatic tumors, push for early diagnosis of pancreatic cancer, improve the prognosis of pancreatic cancer with the multi-disciplinary treatment centering on operation with the aim of improving the five-year survival rate after operation by 10% in the next three years, improve the patients’ quality of life and reduce the treatment cost as much as possible,  strengthen the fundamental research on pancreatic cancer, seek project funding of various sources, establish systematic regulations for the collection and storage of pancreatic tumor specimens relying on the central laboratories of Department of General Surgery and of the hospital, carry out the development research of vaccine for pancreatic cancer and research on the neuronal migration mechanism of pancreatic cancer with the goal of breakthrough results.

The predecessor of the center was the professional group of pancreatic tumor of Department of General Surgery, which was established in 2001. It is now one of the major centers for the treatment of pancreatic cancer in China. Its support department is the Department of General Surgery of Zhongshan Hospital, which is a key discipline of Ministry of Education and a key department of national “211” project. Under the leadership of the hospital and the department leaders and with the joint efforts of all the Center staff, we have achieved significant progress in the treatment of pancreatic cancer in the last 5 years.

Our patient and surgical volume has been increasing every year. The resection operations increased steadily and now remain at a stable rate of 70%. We perform standard pancreaticoduodenectomy, extended pancreaticoduodenectomy, reconstructed pancreaticoduodenectomy combined with vessel resection, distal pancreatectomy, and combined resection of abdominal viscera including pancreatectomy. As our skills improve continually, the incidence of complications is decreasing year by year, with the incidence of biliary fistula and pancreatic fistula consistently below 3% and the five-year cumulative perioperative mortality of 1.4%--far below the average rate of 3%--5% reported in publications. The one-year survival rate after operation is 60% and two-year survival rate after operation is 40%. We have undertaken one key project of fundamental research funded by Science and Technology Commission of Shanghai Municipality, one project funded by Shanghai Municipal Commission of Health, two funded young people’s projects of Fudan University, one graduate student innovation funding project of Fudan University and the total funding is 280, 000 yuan. The members of the pancreatic tumor specialty group have published, as the first authors, 45 papers in Chinese key journals (18 of them were published in magazines of national level) and 5 English papers, among which 23 are concerning pancreatic cancer. We have also jointly published 8 papers on pancreatic cancer with colleagues of other departments. We have attended three national pancreatic surgery conferences and one national annual surgery conference, published 32 conference papers and delivered 3 conference speeches. We have given four academic lectures in national education classes, which improved the academic status and expanded the influences of Zhongshan Hospital. We have cultivated 2 students with doctoral degree and 2 students with master’s degree. Currently there are 2 doctoral students and 3 graduate students studying in the center.
